How to add custom fields in reward treads?

Hello Friends!
i want to modify reward treads, and add a custom form into it,
in other word, a reward tread that look like a form, a combine pic 1 , pic 2!!!

please help me!

i created a form (like pic in post #1) but i want to merge these two function: 1- set reard 2- costum form

in that instruction , we have a form without reward option (i cant find it),

i want create a "REWARD"+form,
please help me.
